Case Management |

Case Managers/Resource Coordinators coordinate services across all program and agency lines. Following the Individual Plan for adults and Individual Family Service Plans for children and families, the Case Manager/Resource Coordinator coordinates and monitors the services to ensure that the person's needs are met. Case Management also determines eligibility, assists in transitions to and from school programs and manages the list of persons waiting for services. |

| Case Managers/Resource Coordinators also are responsible for enrolling individuals into services. For many this involves enrollment into Medicaid Waivers and yearly assessments to determine continued eligibility for these programs. | ||
