Case Management

Case Managers/Service Coordinators coordinate services across all program and agency lines. Following the Service Plan for adults and Individual Family Service Plans for children and families, the Case Manager/Service Coordinator coordinates and monitors the services to ensure that the person’s needs are met. Case Management also makes a developmental disability determination, assists in transitions to and from school programs and manages the list of persons waiting for services.

We are one of 20 Community Centered Boards in Colorado and serve the Western Slope counties of Montrose, Delta, Gunnison, Hinsdale, Ouray and San Miguel.
